Finances and Insurance
Office visits:  Payment is expected at the time of the visit. Dr. Marx is a provider for the following insurance plans only: Medicare, Vantage Health Plan, State Employees Group Benefits, and Blue Cross Blue Shield of Louisiana. You will be given an itemized service statement to assist you in being reimbursed by your insurance company.

Surgery:  As a courtesy, we do file with your insurance for surgical procedures. Please make certain that we have a copy of all insurance cards prior to your surgery. The office staff will do any pre-certification required by the insurance company.

It is our office policy to have a deposit paid prior to the date of surgery. It is also our policy to have any portion of the surgical fee that is not covered by insurance paid in full prior to the date of surgery. Your insurance will be allowed 45 days to pay the claim. After that period of time, if the insurance has not paid, payment will be expected from the patient.

We feel it prudent to remind you that even though you may have insurance, the ultimate responsibility of payment remains solely with you, the patient. Therefore, we ask that you keep informed of the status of your insurance claim and help to see that the insurance company honors your claim in a timely manner.
